Through the Companion Card program, NDS is providing cardholders with a limited time offer for a free Master Locksmiths Access Key (MLAK) to access Changing Places facilities in Western Australia.

Changing Places facilities are secure, accessible toilets with an adult change table and hoist. We encourage you to promote this offer and speak to the people you support as appropriate.

Organisations

If your organisation requires a key(s) to support people with disability who require access to Changing Places, please submit enquiry/feedback with the following:

1. number of keys required
2. contact person and organisational name
3. postal address

Existing Companion Cardholders

For people you support who are existing Companion Cardholders, please complete the online form.
Note: you will be able to complete the form more than once for multiple clients.

New Companion Cardholders

For people you support who are not yet Companion Cardholders, they may wish to apply for a Companion Card and will be able to select a checkbox to receive an MLAK key.
